About Constellation Health Services
Constellation Health Services (CHS) is a family of companies (LLCs) comprised of Constellation Home Care, Constellation Private Duty and Constellation School Based Therapy. We serve the communities of Fairfield and New Haven counties by providing an array of skilled nursing and therapy services, behavioral health, cardiac recovery and home health aide through our certified agency as well as private duty services including personal care, companion and homemaker assistance and geriatric care management. Our School Based Therapy company contracts with 7 area school districts to provide occupational and physical therapy to children from age 3 to 21.
